The report cover sheet you should use depends on your entrance year.

Tear off the receipt (student’s receipt) at the bottom half and submit it together with the report. The receipt will be stamped and returned to you.

Notes on Examination in Essay or Report Format

  1. Please note the following when submitting an essay or a report.
    a.
    You must attach a prescribed cover sheet to the essay or report.
    b.
    The cover sheet and the receipt of report must be filled in with pen.
    c.
    You must submit the essay/report in person, presenting your student ID card. Submission by mail will not be accepted.
    d.
    No essays or reports that are overdue will be accepted. However, if it has become apparent on the due day that you will not be able to submit it by the due time due to an unforeseen accident or other circumstances beyond your control, you must contact the place of submission before the due time and follow the instructions.
    e.
    Once submitted, no additions or corrections may be made to the report even if there is time left by the deadline.
    f.
    You are advised to keep the receipt of report until you receive your grade for the subject.

    If the place of submission of the report is an administrative office, the office will accept reports during its opening hours. However, if a particular due time is specified, the office will accept reports till that time.
    Opening hours vary with each office and day of the week. Please also note that opening hours are subject to change during recess and other times of the year (changes will be posted on message boards in advance).

  2. Dishonesty in essay/report writing
    The following dishonest acts in the preparation of an essay or report will be treated in the same way as cheating in written examination, and are subject to penalty according to the Undergraduate General Regulations should it be confirmed by the Faculty Council.
    a.
    Copying a part of another person’s work or information on the internet without references or indication of quotation in the preparation of an essay or a report and pretending it to be your own writing.
    b.
    Submitting an essay or a report written by another person pretending it to be your own writing.
    c.
    Submitting an essay or a report written by another person upon your request pretending it to be your own writing.
    d.
    Writing an essay or a report for another person upon request.
    e.
    Receiving an essay or a report written by another person for the purpose of copying it, or providing an essay or a report you wrote to another person for the purpose of copying.
    f.
    Other acts that damage the fairness of essay/report writing.

4. Makeup Examination ・・・ Examinations arranged for those who were unable to take the final examination or additional examination approve by the Faculty Council due to illness or unavoidable reasons.

Please note the following if you wish to take a makeup examination.
a.
You must submit the request for makeup examination (available from the Certificate Issuing Machine), to the Center for Academic Affairs or the office of faculty you belong to (hereinafter called “the Office”) within three days (excluding days when the office is closed) after the examination for the course.
If the absence is due to extracurricular activities, you must submit the request for makeup examination and receive approval for it prior to the examination concerned.
b.
It must be accompanied by a document certifying the reason of absence
(see the following table).

※When we cannot judge it about a reason targeted for the makeup examination , we post it to refer to a career center.

c.
The makeup examination fee is 1,000 yen per course (non-refundable).
However, this fee will be waived for makeup examinations arranged due to participation in the Saiban-in (Jury duty) system and the Committee for Inquest of Prosecution system.
d.
Makeup examinations are evaluated in the same way as regular examinations. There are no point deductions for taking makeup examinations.
e.
There are no makeup examinations offered for examinations in essay or report format. However, if it has become apparent on the due date that you will not be able to submit the report on time due to an unforeseen accident or other circumstances beyond your control, you must contact the Office before the deadline and follow their instructions.
f.
If you are unable to a take makeup examinations for some reason, there will be no additional makeup exams offered.


The request for makeup examination form can be issued at the Certificate Issuing Machine (the makeup examination fee is required. (★)) In case that you apply for a makeup examination due to participation in the Saiban-in system and the Committee for Inquest of Prosecution system, the free form is available at the office where you submit the form.The form must be filled out with a pen in your own handwriting. You are required to present your Student ID card when submitting the form.
  
After you log in the Certificate Issuing Machine and click the “Fee payment 1” tab, a button with the sign of “Makeup Examination” will be displayed. Click on it , then the menu for makeup examination fee will be displayed. The request for makeup examination form will be printed out after you pay the makeup examination fee.

Commuting time is calculated based on the minimum time required using public transport in principle, and the office of your affiliated faculty or the Center for Academic Affairs on the relevant campus will decide whether a makeup examination may be arranged in your case. If you have any questions, contact the office of your affiliated faculty or the Center for Academic Affairs on either campus in advance.

Notes on Examinations


  1. You must fulfill the following conditions to attend an examination.
    a.
    You have officially registered for the subject.
    b.
    You have fully paid your school fees.
    c.
    You fulfill the requirements set out by the lecturer of the subject concerned. The lecturer may disqualify you from taking the examination if you have not attended more than two thirds of the classes held for the subject.

  2. You must observe the following rules in the examination room.
    a.
    You must take the examination in the designated room.
    b.
    You must bring your student ID card and place it on the desk during the examination. If you forgot to bring it, you must obtain a temporary student ID card at the office before the examination.
    c.
    You must put away all personal belongings in your bag except for those permitted to be used during the examination. Mobile phones (including smartphones and PHS), computers (including mobile and tablet computers), portable music players and smart watches and other portable recording and playback devices (hereinafter collectively referred to as “portable devices”) must be turned off before putting them away in bags (no portable devices are permitted even for checking the time).
    d.
    Those who arrived more than 15 minutes late will not be allowed to take the examination. You cannot leave the examination room until 30 minutes into the examination.
    e.
    Your student ID card and the name on your answer sheet will be checked by the invigilator.
    f.
    You must use a pen to fill in your student ID number and name on the answer sheet.
    g.
    You must fill in your student ID number and name on the answer sheet and submit it even if you give up the examination unfinished. You must not take away the answer sheet home.
    h.
    You must follow all other instructions of the invigilator in the examination room.

  3. Your answer sheet may be deemed invalid if:
    a.
    You did not fill in your name.
    b.
    You did not take the examination in the designated room.
    c.
    You made corrections to your name.

  4. The following acts in the examination room will be considered as cheating.
    a.
    A different person than the student assigned an exam taking the examination impersonating the student .
    b.
    Writing notes on the desk, clothes or your body.
    c.
    Using items permitted in the examination room for a purpose other than intended use, such as writing notes on them.
    d.
    Bringing in cheat sheets or reference books/notebooks not permitted in the examination room, peeking at other examinees’ answers, learning the answers from another person.
    e.
    Telling the answers to other students, or other acts that benefit other students during the examination
    f.
    Talking, showing answer sheets to each other or exchanging answer sheets between distribution and collection of answer sheets.
    g.
    Disobedience to the invigilator’s instructions.

  5. The following acts in the examination room may be considered as cheating.
    a.
    Answering questions using items that are not permitted during the examination.
    b.
    Starting answering questions before the invigilator announces the start of the examination.
    c.
    Holding a writing tool or an eraser or continuing answering questions after the invigilator announced the end of the examination.
    d.
    Any act in the examination room that disturb other examinees.
    e.
    Carrying a mobile phone (including smartphone and PHS), computer (including mobile and tablet computer), portable music player and smart watch or other portable recording and playback devices (hereinafter collectively referred to as “portable devices”) with yourself or keeping them in places or conditions other than as instructed during the examination.
    f.
    Having a portable device or a watch make sound (incoming calls, alarm, vibration etc.)
    g.
    Other acts that damage the fairness of the examination.

  6. If your act during an examination is confirmed by the faculty council as an act of cheating, you will fail (students enrolled in or after 2004) or receive 0 points for (students enrolled in or before 2003) all the subjects you have registered for in the semester concerned, and the decision will also be publicly announced. (Certain subjects as designated by the faculty council are excluded from this measure.)

Notes on examinations using e-learning and other Learning Management System (LMS)

Please note the following when you take an examination using e-learning or other LMS.

a.
Do not tell your password to another person or lend or borrow the IC student ID card.
b.
Impersonation through one of the above methods is considered as cheating and is subject to penalty as in regular written examination should it be confirmed by the Faculty Council of the faculty concerned.
